Kristine M. Talarski

Kristine Talarski is the Life Care Coordinator with Weatherby & Associates, PC.  She received her BSW in Social work from St Joseph University.  Kristine received her Master’s Degree in Social Work from Fordham University.  She has worked in the healthcare field for over 25 years.  Her professional background includes case management and discharge planning for Hartford Hospital, and Hospital for Special Care.  

Kristine has an extensive experience with healthcare and living arrangement issues for the senior population.  She also has many years of experience working in the community with clients and assisting them with obtaining services in their home when she was with Hartford Healthcare at Home as a medical social worker as well as with Connecticut Community Care, Inc. (CCCI).   Her professional experience also includes that of being a nursing home social worker.  At the time of a tragic fire at a local nursing home in Hartford, she took a vital role in providing grief counseling to the staff, residents, and families where she was employed as the Director of Social Work.

She completed her graduate internship at Institute of Living where she co facilitated a cognitive behavioral adult outpatient day program and also managed a caseload of clients providing individual psychosocial cognitive therapy.  As always focused on working with older adults with physical, cognitive, and mental health conditions and their families.
