Home Care Options Offered by the State of Connecticut

Countless older adults have voiced strong desires to remain living in their private homes for as long as possible. They dread the need to rely on supports offered at nursing homes or as they are more commonly known, “skilled nursing facilities” or “SNFs.” The State of Connecticut Department of Social Services has developed a program designed to provide necessary home and community-based supports with the overall goal of keeping older adults out of long term care settings.

The program is called the Connecticut Home Care Program for Elders (CHCPE). Eligibility criteria include being age 65 and older and a resident of Connecticut. Additional eligibility involves financial and care needs. An assessment is conducted to determine if there is a “risk” of nursing home placement. Risk is determined by the need for assistance with activities of daily living (ADLs) such as bathing, eating, and dressing. Income and assets are also assessed. The combination of financial and health care criteria is assessed in order to assign the category of care each recipient receives.

Hourly care provided in the home to recipients is largely done through agencies which have been vetted by Allied Community Resources. Based on state-contracted initial and ongoing health care assessments, agencies will provide caregivers in the home to offer personal care and safety assistance. Some families may choose to hire a private caregiver, and Allied will help with the payroll process for that caregiver.  Funds can also be used to help pay for local adult day centers.  For some who need to reside in communities which offer assisted living services such as medication management, there is a pilot program within the CHCPE that can provide funds to help cover that expense. These are all efforts which are intended to enhance a recipient’s overall quality of life and to keep them living in their desired environment.
